Create a report with On Hold reasons and Close reasons how can i create this?

I need to create a report where i need to track for all tickets that are in State:

 

- On Hold

- Resolved

 

to include the reasons for "on hold" and "resolved" options.

You can just add these two states in the report filter like this.

As @AnveshKumar M  has stated, just create a report based on e.g. the incident table with the condition state is on of on hold, resolved. After you have done this, you can add the column "on hold reason" and "resolution notes" to your report - or is something differently that you're looking for?

is it possible to measure a multiple usage of different on hold reasons chosen in one incident?
meaning is it doable to use choice list table to get all of the on hold reasons selected in one record?

If there has been multiple on hold reason in one ticket, then your best option is to go with a metric definition to catch the different on hold reasons.
