Create incident UI action

I am not a developer by any means, and so I need help! I need to create a UI action on the HR case table, where users can create an incident. When I click the ui action, nothing happens. This is how it's currently configured. What am I doing wrong?

Hello,

you have written server side code in ui action,please uncheck client option and remove first line from code.
