Flow Designer - adding a checklist to a HR Task form

Can someone please help me add a checklist to an HRTASK form using Flow Designer?
I have referred to the following article, but it does not display well.

Hi,

Checklist is below the Activity Stream. Can you try moving the checklist field above worknotes or so once.

Thanks for the reaction!
I placed the checklist on top of the worknotes, but nothing has changed.
