SLA triggering twice on same incidents

Hi All,
I have an SLA triggering twice on some incidents and can't narrow down the cause of this (it only seems to be happening on a sickness absence form and not consistently)
When the case is closed complete the first SLA timer stops and the next one seems to start at the same time but can't be closed out
I have not yet been able to replicate this in a non production environment so am unable to narrow down any possible causes
I have attached the Start and Stop conditions here if anyone has any advice or guidance on what could be causing this
