User without access to form appears as 'Created By' in Email Logs for HR Case

We have an Employee Exit form in our system. Whenever a user submits this form, an HR case is automatically created. However, we observed an issue where, in the email logs related to the HR case, the ' Created By'' field shows the name of the user who doesn't have access to this form. How can I pinpoint the specific script that generated this email log entry?

the sys_created_by will always be that person who is exiting and submitting the case
It's an OOB platform behavior that whoever submits that person's user id will be set in sys_created_by
