How do I embed an image in an Outlook Actionable Message Satisfaction Survey email?

I have recently stumbled upon the Outlook Actionable Messages and worked through the instructions on how to set this up for our Satisfaction Survey. What I have been unable to work out is how to get some branding in the message?
I can see the image appear on the survey definition screen.
And have added it in the source code like this:
But it doesn't appear on the survey email.
What am I doing wrong? We are on the Vancouver release.

Not round here it isn't! We have always known it as SNOW and will do so until it's put out to pastures new.

Not great news! I am not sure of the expertise of the SNOW support engineer I spoke to, but it doesn't seem that there is any documentation on setting up a Survey using the OAM definitions. I was advised to reach out for consultancy which seems a little frustrating seeing as the survey works with the existing set-up, it just doesn't have any branding on it.
Has anyone else stumbled upon any instructions?

The problem what I see with OAM definitions is that you can only setup a new definition for an Approval table. so any other table that are related to an approval, like Requested item, change request , demand, etc.. you can create an OAM definition but what about Survey, you cannot use that.
it must be somewhere else you need to change. I tried to add some layout under the Introduction & End Notes but the HTML is not rendering in outlook.
