How to create a survey which can be send to two users

Hi Team,
Am trying to send a single survey to two users(reported by and reported for ) once the incident is resolved. Can I know how this can be achieved

Hi,
In reference to the out-of-the-box (OOB) functionality, it is not feasible to achieve your request. The justification for this is that the OOB workflow permits the distribution of surveys and assessments to a single individual. You will need to configure the item using the OOB form. but can i know if i create 2 trigger condition in the survey it is triggering two assessment instance for two users, but only the first person is able to take the survey. Why the second person who is assigned with the assessment instance is not able to take it up
