Does anyone have Service Mapping requirements gathering questionnaire (various questions we put to architects, business owners, stakeholders, network, security and database teams) model document in handy?

We are planning to Implement Service Mapping for the Business Service Applications in our Organization, it would be clearly helpful if you can share the existing requirements document.

similar to the question above ,  implementing discovery is a prerequisite to service mapping  or is service mapping can be implemented without setting up infrastructure discovery?

as far as i know... discovery first and then comes Service Mapping.

I'm actually presenting at K18 about this very topic and how it relates to service mapping.  Bottom line: you can service map without performing discovery, but you don't want to. Service mapping will use discovery probes, sensors and patterns to identify and create CIs for anything that's not already discovered. So service mapping before you discover and work through your network will slow down service mapping and complicate troubleshooting issues (e.g., are they mapping issues or discovery issues).

Lesson learned - discover first, map later. 😉

Since it didn't look like anyone responded, I've attached the template we're using. It's a ServiceNow template. It's worked for us.
