Duplicates Cloud Host CIs Getting Creayed on Each Discovery Run

Hello everyone,
We are facing an issue with duplicate Cloud Host CIs being created during each Discovery run. The CIs discovered have the same Name and Object ID, but instead of updating the existing CI, Discovery is creating a new one every time.
The CI Identifier for the Cloud Host class currently includes only two attributes: Object ID and Name.
Has anyone encountered a similar issue?
1. What could be causing Discovery to treat each run as new CI instead of updating the existing one?
2. Where can i start troubleshooting to identify the root cause?
3. Are there additional identifier rules or configurations we should consider?

Check if any inclusion rule created for the CI. You can do that by going to ci class manager - > class -> click on identification rule and scroll down.
Also confirm if the install status changing for these CI's
