Service request form on SNOW

I have created a Service request form on SNOW however the Catalog task is not getting created automatically upon getting the RITM Approved.
I have checked the Workflow part seems to be fine.

image001.jpg shows you where the problem is. Your opening IF statement isn't evaluating to be true, and that directs the workflow to end.

Hey Soni, are you the only SN resource for this account?
We can give you suggestions but if you haven't been working in workflow before, this can be pretty risky.
Before we proceed, are you in Dev and do you have an update set open? If the answer to both of those questions is yes, then open up that IF activity and paste its code onto this thread.

Please click on "Show workflow" under related list to check where the activity points to once your request is created.
