Service request form on SNOW

I have created a Service request form on SNOW however the Catalog task is not getting created automatically upon getting the RITM Approved.
I have checked the Workflow part seems to be fine.
Please Help

It looks from your comment that the request form needs value for field application.
Can you confirm if there is any field for application values and has a red slash beside the field name. If possible please post the screen shot.

i craeted a variable in a variable set. After that when i select that variable in the form and hit submit it gives me an error

We're not sure what you mean by "select that variable in the form"
