Adding form for assigned user input to SCTask

I'm stuck on this scenario and can't figure this one out.

 

We're getting a new hire in and HR creates a RITM for ICT to set everything up.

 

Several departments are assigned SCTasks for this new user. IAM needs to return info like login credentials and email adress, hardware needs to return PC number, etc...

 

What's the easiest way to request these kinds of information. Is there a way to add a form to SCTask or is there another way I'm not seeing?

Do you mean you need this information from user or do you want to pass this information to other groups via task?

sc tasks are the tasks which can be assigned to different groups and they can work on it.

You can have variables defined at catalog level and don't show them during catalog form submission

Show them only on Catalog tasks form so that agents from that group can add/update the information
