Assessment instance record not getting inserted

After clicking "Assign survey" UI form button Assessment instance(amt_assessment_instance) record not getting inserted, throwing error message like shown in image.

Yes Vamsi, I found that There is no record in category in Assessable records, after editing record manually assessment instance is getting created.

But how to make that assessable record inserted in category automatically?

Can you confirm if you have atleast one active category under assessable record and one active assessment metric under category?

Before under category it was empty, later i added Test Survey 1 as show in image
