Assessment Trigger Condition is not working as expected

I am creating a Trigger Condition against Business Service Table and wants to trigger an Assessment(custom made) to Owner of Business Service Table when the Business Criticality field changes to "1- most critical".
But as per the condition the assessments are not getting generated to the owner of Business Service.
Also any insights on "Assessable Record Field" on trigger condition form. Is this the reason why assessment is not getting generated

Hello,
Keep repeat interval as 0 instead of 30.
Remove 'itil' role and add following 'survey_admin,survey_reader,public' remove public role later on or do according to your need

I have tried following above instructions but the result is same, my assessment is not getting triggered.
Is this due to Assessable Record Field??

I also got error:
Cannot create assessment instance, null was detected from assessable record field.: no thrown error
