Bulk changes to CI

I am looking to use the Bulk CI changes feature but I am unsure how the Proposed Change fields are supposed to be added to the change form.  

Within the ServiceNow documentation it refers to how to configure CI changes and gives an example of how Proposed Change and CI class fields are visible on the form.  See screenshots below.

Ca guidance be provided to how we can set this up since in the practical sense this cannot be custom fields (for Proposed Change fields) since Change Management can lead to multiple ways in which a CMDB is updated.  Potentially, I am missing an important step in its procedural usage but any guidance would be greatly appreciated.
