Change Request Requirements document creation

Hello all, I recently joined a company and started a new project with a client for Change Management.  The project involves implementing standard change request and risk calculators.  As part of initial deliverable I have been asked to produce a functional, business and technical requirement document (excel) for the implementation.  Does anyone have any templates or samples that I can leverage.  I can make configuration changes and able to show the demo of the standard change, but little lost in creating the detailed functional specification document.

Really appreciate any help in this regard.

Hello Jay K,

I think you should go to Now Create and pick the Success Pack related with IT Service Management.
That one will have plenty of documentation (guidelines, workshop data, requirements, etc) for the Change Management process.
