Hi all - Hoping somebody out there has experienced something similar or is able to provide some guidance.
Summary
Two risk assessments have been configured for users to complete during the change process, one during the assess state and the other during authorize. The rationale for two assessments is that risk is hard to define at the start of the change process, but more established prior to deployment. Now, the issue that is being experienced is only the last completed assessment is recorded in the Risk Assessment related list pane (retrieval from task_assessment table).
This could easily be worked around with a report given all assessments are saved into survey_instance and survey_response... however, this table does not record the source (change record number) like in task_assessment, thereby not being able to tie back to the change number for reporting.
So, my question is, is there anyway to either record all assessments that are completed in the task_assessment table and thereby show on the Risk Assessment related list OR associate the change number to the underlying survey_instance table?

Hi Nathan,
I'm sure there could be some business rules or other artifacts that could be disabled that may be enforcing having only 1 risk assessment at a time for a change. Now, objectively looking into the situation, having only 1 risk assessment against a change request is what makes the most of the sense; otherwise, if you have more than 1, which one is going to be the right one? the last one right?
Have you considered only requiring your users to fill the survey assessment once at the right moment, prior the deployment?

Thanks for the reply Berny. I don't necessarily disagree with you in regards to having a single risk assessment. However, the requirements of the customer were very specific about being able to capture initial impact early on via set questions in regards to the scope of the change then later on deployment risk in a separate questionnaire. The rationale is then for the Change Approval Board to consume both sets of responses as part of their decision making.
I'll take a look at the script includes as part of the plugin as per the available documentation Script includes installed with change risk assessment

I am also looking for similar kind of thing. Our client also requires multiple assessments in different stages. Please share the solution if found to display multiple records under Risk assessment related list.

Hi Nathan,
I have a request to create multiple assessments as well - very similar to your ask.
Before I even get to showing the results of multiple assessments, I'm trying to figure out how to have them created. If you would share your solution highlights, that would be terrific.
