If we need to create a single form on the portal to raise cases in different custom tables, what approach do we follow for this, and what should be the table name when we create this generic form

Use the choices or categories on form and based on same call the different tables in back end.

This is exactly what a 'Record Producer' is for...

Record Producers are a type of catalog item used within the Service Catalog to allow users to create records (such as incidents, requests, changes, or cases or customised records) through a user-friendly interface. It's an efficient way to simplify the record creation process.
