Embedding Survey in an Email Notfication

Hi I am new to ServiceNow learning Surveys. I have created some simple smilies on "How was your experience?" which approach would I need to take to get that survey on the email notfication. I have attempted it but no luck in doing. Would appreciate the guidance and ideas, thanks.

Open The survey you have created
Use Trigger condition to decide the receiver and Trigger of the Survey:

1. Set table,
2. User Field(Can be caller)
3. Set repeat interval to 00:00:00
4. Set the Trigger(State --> is --> Closed)

This should trigger the survey.
It auto populates the information in Survey and Assessments in OOTB SP portal too.

I want the survey " How was your experience?" at the bottom of the email body when an incident is resolved. Its more about how do I get the survey on the email rather than have another notification sent to them to do. Its should 1 email not more.
