Survey embedded Email with emoji

I wanted to embed a small survey with two emojis of happy and sad, how to achieve this and in which table the data should be stored. 
The survey should be only two clickable buttons which for happy and sad, and that should record the response in a table for which we can also create a report. 

I have created a survey so can i create a survey response instance with the code on click event of the button.
