Embedding Survey in an Email Notfication

Hi I am new to ServiceNow learning Surveys. I have created some simple smilies on "How was your experience?" which approach would I need to take to get that survey on the email notfication. I have attempted it but no luck in doing. Would appreciate the guidance and ideas, thanks.

Its one email and should give requestors the option to fill on the email rather have a link to the survey.
