We have a requirement for users to provide feedback on how our department is running especially with the recent implementation of ServiceNow (Madrid) and our new Service Portal.

I have created a simple survey with one question, similar to the above picture you have got above.
I then added a new ICON link to the survey. I named it 'Provide Feedback'.
When the user clicked the link widget it would re-direct them to the simple survey and they could submit that
