Flow Designer executions stuck approvals not triggering.

Hi everyone,

We’re experiencing an ongoing issue with Flow Designer , where multiple flows are stuck in In Progress or are not moving past an approval step.
flow context of RITM also showing the error as you can see in the image.

so it's stuck at approval?

Did approval record get inserted for that?

Only the RITM is being created, but it is not moving forward for approval.No approval record

Flow is in "waiting" state even though approval or wait for condition has been satisfied
