Flowdesigner Lookup record condition returned false even if the record is present

Hi,

  I have a custom table which have users record with other details. I have written a flow with trigger condition as 'Active flag changes to false', and other actions will lookup this user record in custom table and change the status of this record to 'Leaver'. If i test this manually it is changing the status and creating the RITM but still have same 'No Record found' error on flow execution.
