Does anyone know of any good documentation on setting up a Follow-the-Sun On-Call scheduling? The documentation talks about it being possible, but actually setting that up is missing or very difficult to find.

Yes you can.
You have to create a separate shift for each one. NA, EMA, APAC, etc. In each of those shifts create a schedule for their daily start time and end time of 8 hours. I did it by creating the first one and then after I got it setup did an insert/stay so I could create APAC and EMEA and then created their schedules. It's working great.

I haven't done much with On-Call in several years but I remember an out of the box example being provided for the Network team. It has EMEA, Americas, and APAC setup in the demo data. If you load the demo data from that plugin or use a Personal Developer Instance you can view it.

There is a lab guide from the 2013 Knowledge conference titled "Automating Incident Management" that provides a step by step guide to setting up a follow the sun process.
Now the trick will be finding a copy.

Wow you are bring back very old memories. Unfortunately that lab wouldn't apply given it was pre-Geneva which was the release where On-Call Scheduling had a major update. That lab was very script heavy if I recall correctly.
