Does anyone know of any good documentation on setting up a Follow-the-Sun On-Call scheduling? The documentation talks about it being possible, but actually setting that up is missing or very difficult to find.

Yes you can.
You have to create a separate shift for each one. NA, EMA, APAC, etc. In each of those shifts create a schedule for their daily start time and end time of 8 hours. I did it by creating the first one and then after I got it setup did an insert/stay so I could create APAC and EMEA and then created their schedules. It's working great.

It's still of use, even on Madrid and New York.
I recently used part of the lab guide to setup a "round-robin" auto assignment for our Service Desk based on a system schedule. It's working pretty solidly in New York.
