Hi Srinu,
This requirement can be done in 2 folds.
Firstly, you will need to cerate a new change model.
Navigate to Change> Administration > Change Models > Click on "New"
Open one of the "Change model" like "Normal" for your own reference .
Then , fill out this form :
Now, Navigate to System Definition> Interceptors
Look for " New change Request" and open it:
Now, under "new change request" form, you could see "Answers" in the related list, there you will need add an new answer , click on "New"
Mark my answer correct & Helpful, if Applicable.
