How to Display  sp Modal Box or Text Box When click Reject Button  at Approvals Widget?

Scenario : When Click View All Approval at  Approval,  Displaying  all approvals or requests ,Here when Click Reject Button Displaying sp Modal Box(Mandatory)  or Text Box (Mandatory) and  enter comments.  Comments Mandatory for  Reject . And Sharing screens, Please help me ..  Thanks
