I want to make a survey on a custom table with below inputs.

Hello Karthik,
In jakarta and istanbul, you have "my survey" widget which will show list of all surveys which are assigned to the end user in the service portal

Hi Karthik,
I would recommend using new Assessment instead of the Legacy Survey. Assessment, Quiz, and Surveys are similar and they internally use the same framework. You can create an assessment on any tables (including Custom tables).
As the new Survey & Assessments are enhanced for every ServiceNow release and Legacy Surveys are not, please avoid using Legacy surveys as much as possible.

Hello,
I would also prefer the Assessment. However, for our external customers where the email field only resides in the custom table, we have only found a way to use Legacy survey. Do you have a recommendation on how to use the new survey where the email is not available in the User field?
