How to make Risk Assessment Mandatory for change Request

Hi All - we have Risk Assessment for change management and the questions are not mandatory.  Whenever user clicks on Risk Assessment, a form opens and without answering any question user can close the form assuming the risk assessment is completed. How can we make sure user submits the answers for all questions.
