how to resist visibility of report on other forms

on survey, I have created a survey form but I want to configure a chart but it is reflecting on other survey forms also how can resist this without showing on other forms

i have created a survey form on (assessment and survey module ) and i am triggering it on an Ritm ,when any customer makes the RITM Accept solution  then the survey will triggerr and they will give the feed back. now i want to populate the report on the survey form using chart  but while i am making changes on form design layout and capturing the data this chart is reflecting on other survey forms on (asmt_metric_type) table . how can i resist visibility of the chart for that form itslef.
