m2m related list not added as related list

I created a new m2m relationship between change and problem. It works fine in DEV when i move to test I am unable to add it to the form using Related lists. I see that when I add in the right side but it does not appear on the form. How can I fix this?

Added Problems to the right and saved. Still on the form it doesnt show up
