Missing Resolution Notes and Resolution Code on Field Changes note

I have been unable to work out why our Instance does not copy the Incident Resolution Notes and Resolution Code onto the Field Changes note when an Incident is Resolved. All we see is the State Change.

I imagine this is very simple but I cannot see any business rules that control this. Can someone put me out of my misery?

Check what fields you have defined to write to the activity stream and make sure resolution notes and code are selected
