Need help for SC order status widget customization

Hi 

Currently, when the request has been submitted in the service portal, it will be redirected to request summary and will show the RITM fields like "request number, item, stage". But as part of recent requirement, need to include some more fields like state & approval . To achieve this, I have cloned the OOB widget (SC order status) and trying to add the script but no luck. could anyone help on the customization/script where exactly it needs to be updated
