i have a multiple check box under labels in catalog form so which check box is selected i should get it in email notification

These are just variable values, not the checkbox for notifications. Once you submit the request, the email will be triggered automatically.

since those are checkbox type, they will give you true/false value

Do you want the tickbox symbol in email?
