Problem mandatory fields for creation breaks create problem Ui action

Hi all,
We got this requirements that we need to set cause note and fix note mandatory when creating a problem. However, this is breaking the “create problem” UI action in the incident table.
The problem are basically created using the SOW’s incident Ui action.
I tried to create a new problem view and show it using GlideModelForm to get and transfer the value of the two fields but got no luck to make it even show the dialog box.
any ideas?
