Promote to Major Incident 'Promote' button is not working

Hello,

I have an issue that I am not sure where to start. When the Incident is proposed as a Major Incident, it shows up under 'Candidates'. Then, when I try to promote it to Major Incident from the candidate record or just from the Incident record via the UI action ' Promote to Major Incident', the pop-up fields get filled in but the 'Promote' button does not now. It does not promote the incident to a Major Incident. What am I missing here and did any of you experience these issues?

Also, what I have noticed is that the MI tab is missing on the Incident form. I had to manually added it in the form sections module. I have tried to repair the plugin as well and that did not fix it.
