Regarding transfer of Universal Request to different departments

Hi,

I have a requirement that I want to have a "transfer" UI Action on the Universal request record and on click of it should show a modal popup which lets you select the Department out of the choices to transfer the request to that selected department from Universal Request. I need some inputs how to achieve this. 

Any inputs here?
