I want related list of change in request item form and request item related list on change form. We have created UI action on request item form to create change so I can display the number of change under related change and request number under related request

so how are both RITM and CHG related to each other?
you must be storing RITM sysId in CHG since you are creating CHG from RITM?
based on that you can have related list

We have created UI action in order to create change from request once we click on create normal change it creates change

This is pure customization. Out of the box (OOTB), we dont have a UI action to create a change from the RITM. You would need to build the full logic yourself, and I would suggest avoiding this. Instead, it’s best to use the OOTB related list I shared earlier, as that is the recommended approach.
