Request Approvals are showing double in ServiceNow

Hi,

 

We have observed one bug in our instances, when we raise the request. approvals showing double double records for one approver. any one record is approved another one also approving. Why this is happening?

And please suggest me How to investigate this type issues.
If anyone knows please let us know.

Check is any sync is working in backend> or in flow/ WF approvals are getting generated duplicate.
