Risk Assessment issue in change management

Hi,
In production environment some of the changes are facing issue with risk assessment calculation. When the change is on new state and complete the assess risk prior to the request approval. It is still showing the error as "Risk assessment should be done prior to request approval". But for some changes there is no issue and the flow is moving forward. What is the real issue for this happening?

The expected behavior and action is that the Risk Assessment needs to be done before moving the change to the Approval state, not after.
This could be a plugin issue — try to repair the Risk Assessment plugin and check again. Also, ensure that the Risk Assessment is added to the Change form once it’s done.
