Save and Submit buttons not visible on assessessment form

Hi,

We recently upgraded to Tokyo and we see the save and submit button on the assessment form is missing. If anyone is aware on how to resolve this issue. Please let me know.
On the assessment URL i see: #sp_survey_root to be added
 https://<instance>.service-now.com/sp?id=take_survey&instance_id=acac2c2b8727a114f45d873d0ebb354f#sp_survey_root
