As the Service Catalog Process Owner, I would like to send an email based survey to the Service Catalog Requester after 5 business days from the completion of a RITM, so that I am able to manage and track the quality of work.

Have you explored Survey Definition(asmt_metric_type). 

This is OOTB way to do surveys, this should help.
