I have an issue where some Change Managers are creating a duplicate Standard Change Form/Record when they create a new Standard Change via the SC Catalog Template.
This doesn't happen for all users.
He's what we're doing
1. Navigate to [Standard Change]
2. Select Standard Change Catalog
3. Select a Catalog category, e.g.
4. Select a Standard Change Template -
5. Note the Change Request number allocated - e.g. CHG4019204
6. Enter a Planned Start Date of today/now
7. Enter a Planned End Date of today/now
8. Select Save
9. Navigate to [Change], then [Open]
10. Check to see if Change Request has created just the one entry or duplicate
The highlighted rows show the duplicate records. (the defect creates a duplicate record one second before the 'correct' record, but does not capture the planned start/end dates or the assignment group which is mandatory set in the template)
I've logged into our Test instance on my device using their identies (not impersonated, but changed passwords and logged in directly as them) and I don't get the duplicate records.
I've logged into our Test instance on their devices and do get the duplicate records.
Change Manager has logged onto Test instance on my device and doesn't get the duplicate.
So it doesn't appear to be device specific or user specific, but the combination of user on their machine triggers the bug??
As I understand Standard Change Records are produced just by opening a RFC from a template, i.e. you don't have to save/update the record for a record/change ref to be generated.

Users were found to be double clicking on the Standard Change Template.
This created the 2 Standard Change records immediately after one another

Are you using a catalog item workflow for these standard changes? If so, what does that look like?
