Survey Is not triggering with proper subject name and survey Link

Hii,

 

There is one oob Notification called Survey Invitation but this survey should trigger whenever  5 th request is created for that I have made business rule that business rule is triggering but whenever I preview the email It is not showing the proper subject and in the body that link for the survey is not showing.
