Survey Notification for Change Request

I have created a new survey for change management that is triggered when a change requests moves from the implementation state to the review state and the requestor of the change is the person that is responsible for completing the survey.

However, there is a default email notification that is emailed to the change requestor to complete the survey. This default email notification is the email notification for incident surveys. How am I able to link a new email notification to the survey I created?

Ok so I am answering this question with the assumption that the email sent actually does contain the correct Survey link and is sent to the correct person- but due to the formatting you set on that notification its better for you to send another notification where you can format it especially for the change survey- the default OOB is very plain and plugs in name of Survey in a one size fits all setup.

Well assuming the above you would go to the condition and add something like and SURVEY is NOT Change Management Survey ( you would have to locate the field it may bot be called Survey) Then on the new notification make sure the condition aims specifically for the change Survey.
