Navigate to asmt_metric_type.LIST and select your survey.
For example, Name: Short Customer Satisfaction Survey using Smiley Face
From the below related list, Trigger conditions
Create New record and use the fields to whom and when it should be sent.
For first question : Select user field : Assigned to, caller, or any ...
For second question: Select the conditions. For Assignment : Assignment group|| changes
You can create multiple records like these.
