workflow Activity its not working

i try to add the one Approval step in OOB normal change work flow , but it was not trigger , how to solve this ? kindly help to me regarding this.

Did you verified your workflow is in published state and active. Is there any condition kept for approval is it getting satisfied?
Can you share screenshot if possible what configuration has been done by you so we can guide you on same.
