workflow Activity its not working

i try to add the one Approval step in OOB normal change work flow , but it was not trigger , how to solve this ? kindly help to me regarding this.

Add the approval in the Chane approval policy.

I should create via workflow only , not using Change Approval policy or flow designer

Agreed, but the best practice for changes is to use change approval policies, as it's a safe and effective approach. Please share where you're facing issues.
