It is not any configuration. When you open Form Builder, it will open forms with yellow icon for which you have attached screenshot. There is a new feature in ServiceNow that is form builder. I'm not sure if it is a new feature within Washington or Vancouver release of ServiceNow because in Utah this option was not available.
Below is the steps to navigate to Form Builder
Go to any form -> Right Click at the top -> Configure -> Form Builder
You will see following form once you opened form builder
